We are writing to inform you of a change to the Citywide Immunization Registry’s (CIR) Online Registry (OR) Website address that your facility regularly accesses.
As soon as possible, and no later than January 11, 2016, please be sure to use the following new URL to access the log in page directly:
<https://immunize.nyc/provider-client/servlet/PC>. This new URL replaces the old URL < https://a816-healthpsi.nyc.gov/provider-client/servlet/PC>. The old URL will remain active only for a brief time after January 11th. Please save the new URL in your bookmarks, favorites or shortcuts now.
You may also continue to go the CIR’s main informational page: www.nyc.gov/health/cir to click on the “Online Registry GO” button:
